Emergency Contact Form
Please fill out an Emergency Contact Form before the start of the summer program session that your child will attend. The form will be sent to you via email before the start of the session. Your child may not attend class until this form is received. You only need to fill the form out once if your child is attending multiple sessions or programs.
No refunds will be issued after May 31,
Before May 31, a $250 cancellation
fee will be deducted from the refund
amount. After May 31, you may withdraw
any time before the start of the session
for a credit minus the cancellation fee.
No withdrawals are accepted after the
start of a session. There are no allowances
for absences, early withdrawals, or late
starts. There is no pro-rating of fees. Additional FIAF policies apply, read here.
Absence and Early Release Policy
If your child must leave early, written
notice from a parent or guardian is
required 24 hours in advance. There are
no make-up classes for absences. FIAF
must be notified if a child will be absent.
Children must bring their own snack and lunch. A list of necessary clothing, arrival and dismissal arrangements, medical forms, and other pertinent paperwork will be mailed via email to parents before the start of the session. FIAF is not responsible for any lost belongings. Please clearly label all belongings with your child’s name.
